A retrieval system needs a lifecycle for knowledge, not just a one-time document import.
Understand what retrieval adds
Retrieval-augmented generation supplies relevant material from a knowledge collection to a model during a task. That can make a system more useful for a particular organisation, but the retrieved material can still be incomplete, outdated or inappropriate for the user. The practical question is not only whether a document can be found, but whether it is the right version for the decision being made.
Treat updates as a product workflow
Record when source material changed, when the change was ingested and when it became available to answers. A hypothetical support assistant should not continue recommending a retired policy simply because an old copy remains easy to retrieve. Deletions and access changes need the same attention as new documents. A document removed from the source should not quietly survive in an alternate searchable copy.
The knowledge maintenance loop
Source
Track authoritative documents and permissions.
Ingest
Apply updates and deletions.
Retrieve
Test the material actually selected.
Answer
Expose evidence and uncertainty.
Evaluate retrieval separately from writing
When an answer fails, inspect the material the system actually received. If the relevant document was absent, changing the writing prompt may not solve the problem. Test queries with exact identifiers, ambiguous terms and recently updated facts. Then assess whether the answer represents the retrieved evidence faithfully. This separates a search problem from a generation problem and makes fixes more targeted.

Give users a way to verify
Show useful source references and, where relevant, the date or version behind the answer. Make uncertainty visible when the collection cannot support a conclusion. Customers should have a clear way to report a stale or incorrect source. A knowledge product earns trust through this maintenance loop: information changes, the system reflects the change and a user can understand what evidence supported the response.
